When curative treatment is no longer a choice or an option, the interdisciplinary team at Cypress Hospice is committed to developing an individualized plan of care that meets the physical, emotional and spiritual needs of the patient and family.

The interdisciplinary team includes:

• Medical Director – Our Medical Director approves and oversees the plan of care for each patient and serves as a consultant to the interdisciplinary team including the primary care practitioner
• RN Case Manager – Our RN Case Manager develops the individualized plan of care including pain and symptom management and is responsible for coordinating and ensuring the continuity of care
• Spiritual Care – Our Chaplains will address the spiritual well-being of patients and families to ensure that their individualized needs are met
• Social Worker – Our Social Workers will address the psychosocial and emotional needs of our patients and families as well as advocate for the patient’s end-of-life wishes
• STNA – Our State Tested Nursing Assistants provide dignified care with the activities of daily living including personal care
• Bereavement – Our bereavement program develops a plan of care to provide support through the grieving process
• Volunteer – Our volunteers provide individualized companionship based on the needs of the patient and family
